Collection of your personal information
I will only collect personal information necessary for provision of a specified service and will collect information lawfully, fairly and not intrusively. Generally, information collected will include:
patient's name, address, telephone number, name of next of kin or emergency contact and Medicare number
treatments or drugs currently used by the patient
past and present medical history including a family medical history if clinically relevant
identity of any health/medical service provider or specialist to whom I refer the patient or who has referred the patient to me
copies of any referral letters and any reports back
I will mostly collect your personal information directly from you over the phone, via the internet or in person. I may also collect personal information about you from third parties such as your referring medical specialist and the hospital or health facility where you are admitted. It may be beneficial in some cases to collect family medical histories from patients.
Using and disclosing your personal information
I may use within my practice or disclose to third parties a person's health information:
for the specific reason it was collected
for secondary purposes that are directly related to treatment if the patient would reasonably expect these
if the patient agrees to the proposed use or disclosure
if otherwise permitted by law
I generally will use and disclose health information to various health professionals for the provision of medical services. I may refer patients to the following services:
public hospitals
private hospitals
radiology services
pathology services
specialist medical practitioners
other health service providers involved in the patient's treatment such as pharmacists, anaesthetists and ambulance services
Secondary purposes directly related to collection of personal information for which I may use or disclose information may be for training, billing, quality assurance, and liaison with private health insurers and government departments regarding Medicare entitlements.

If your personal information is collected for any other purpose than health/medical care and treatment or needs to be disclosed to any other party, I will inform you about why your information is needed, how it will be used and to whom it may be given.
Security of your personal information
I will take reasonable steps to secure and protect your personal information from loss, misuse or unauthorised access. Information no longer needed will be destroyed. Health information may be necessary for future treatment of the patient or for public health reasons, or the law may require it to be retained. If so, your data will be stored securely in hard copy documents or as electronic data in my computer system. I maintain reasonable physical security over paper and electronic data and over my premises through the use of locks and security systems. My computer security ensures passwords are necessary for access to patient files within my computer system.
Access and correction of your personal information
Patients generally have a right of access to their own health records, as well as other personal information held about them. I will only deny access in certain circumstances such as where access poses a serious risk to a person's health or life. If I refuse to provide you with your information I will provide reasons for that refusal and exceptions relied upon under the Privacy Act. If any of the personal information I have about you is out-of-date, inaccurate or incomplete, please contact me immediately and I will take reasonable steps to correct that information.
Maintenance of your personal information
I will take reasonable steps to ensure your personal information is up-to-date, accurate and complete.
Openness and accountability
I am open and transparent about how I handle personal information, including health information.
Identifiers
Government identifiers such as your Medicare number or Veterans Affairs number are not used within my record keeping system. I will only use or disclose these identifiers for the reasons they were issued or as otherwise lawfully permitted.
Transnational flow of your personal information
Personal information will only be transferred out of Australia if laws or a scheme binding on the recipient provide similar privacy protection to Australia's Privacy Act. Personal information will otherwise only be transferred with the person's consent or as otherwise permitted by law.
